2010 State Leadership Conference

Registration Instructions

To ensure that you and your students have a great
conference experience, please review the following information before
you enter the Registration website. Print a copy of this document for
reference as you complete the process.
The online registration is similar to an Excel
workbook. There are five pages and you must enter data on each page. The
entire process must be completed in one session. You cannot save
information and return to complete the registration later. Review each
page carefully to ensure accuracy. Print each page before going to the
next. You will not be able to make changes after you click Submit
on the last page of the registration.
Only students who have qualified to compete at the
State Leadership Conference can be registered. If you have scholarship
applicants who are not contestants, it will be best to wait to register
until after you receive notification of the scholarship finalists, which
should be no later than February 10.

School
Information
- The registration form opens to the
School Information page. Step-by-step
instructions follow.
- Enter the requested advisor information. If
the registration is for more than one chapter in your school, list
the advisor to whom questions should be addressed. Pressing Tab
will take you to the next data-entry field.

- The advisor’s cell phone number is requested
so that we have an emergency contact number while students are at
SLC. The number will not be published or shared.
- Select your school from the drop-down list.
The list contains schools with current membership. If your school is
not listed (not likely), leave the field blank and continue with the
registration.
- Enter the school address components.
- Select your Region from the drop down list.
- Review your entry in each field.
- Right-click and print a copy of this page.
- Click Registration below
the entry fields to move to page 2.
Registration
- Only students who have qualified to compete at
the State Leadership Conference can be registered. Student guests
are not allowed.
- Enter a student’s first and last name on line
1 of Registration – Page 1.
- Select the student’s contest from the
drop-down list.
- Select Yes if this contest is a team
event; otherwise, select No or leave the column blank.
- A contestant who is also a scholarship
finalist should be entered a second time with Scholarship as
his or her contest. Only one registration fee will be charged.

- Continue to enter students who have qualified
for state competition.
- If you have more than 30 contestants, click
Page 2 on the right side of this screen to open a second
Registration page.
- Verify the accuracy of students and contests.
- Right-click and print a copy of this page.
- Click Lodging to move to page 3.
Lodging
Information
The information on
this page will be transmitted to the hotel. Each line on this form
represents one hotel room.
- Select the Room Type (single, double, etc.)
from the drop-down list in the first column.
- Enter last name, first name in the Name1
field.
- Tab to the Name2 column. Enter last
name, first name for the second resident in this room.
- The total number of names entered on each row
must correspond to the Room Type selected in column 1.
- From the drop-down list, select the gender for
the residents listed for the room.
- Select the number of Nights for the
hotel stay.
- In the Rooms column, select the
category for the residents in this room (advisor, students, etc.).

- If a room will be shared with an individual
from another school, select Yes in the Sharing column.
If all residents are from your school, this column can be left
blank, or you can select No.
- In the last column, key any special needs that
relate to the hotel room. (Example: handicapped room required).
- Continue to enter rooms until all who are
registered have been assigned a room. All advisors, adult guests and
chaperones must pay registration and room rates in order to attend
the conference.
- Review for accuracy and print a copy of this
page.
- Click Treasurer to move to page

Treasurer Information
- Enter the number of students who will attend.
This number must correspond to the number of students entered on the
Registration page. Student registration fees will be
calculated in the Total Amount Due field.
- Enter the number of adults (advisors,
chaperones, etc.) who will attend. Adult registration will be
calculated.
- If your school’s attendees will stay Thursday
night only, tab to the Lodging Thursday fields.
- For those who will stay at the hotel on
Wednesday evening, enter the number of rooms of each category
selected on the Lodging page. Calculations will be
reflected in the total due.
- Repeat the process for Thursday night.
- Columbus-area schools whose participants will
commute to conference events rather than stay overnight, must send a
signed statement from the Principal/Director. Fax the statement to
Sheri Ketaneh (614‑728-6176) or e-email a pdf attachment (Sheri.Ketaneh@ode.state.oh.us).
- Enter the Purchase Order Number. If you do not
have a number, enter In Process.
- Enter the district treasurer’s name and
contract information in the requested fields.
- If any registered participant(s) require
special accommodations or assistance for activities at the Hyatt
Regency or at Veterans Memorial, identify the individual(s) and
describe the needed accommodations.
- Review and print this page.
- Click Proctors to move to page

Proctors
·
The number of individuals listed on this page must match
the number of adults entered on the Treasurer page.

For each advisor who will attend, enter Name and
E-Mail.
·
Each registered advisor will be assigned to assist with a
contest. From the drop-down menus, select a first, second, and third
duty assignment request.

Below the list of advisors, enter the names of chaperones
and other adult attendees. No duty assignments will be selected.
·
Each advisor will receive an e-mail confirmation of his or
her contest responsibility prior to the conference.

Verify accuracy and print a copy.
Review and
Submit
·
Registration deadline is February 19, 2010.
·
Carefully proof each printed page. You can return to any
page to make corrections.
·
When you are confident that all information is accurate,
click Submit Registration.
·
A registration confirmation will be e-mailed to the
advisor whose name appears on the first page.
Additional Details
- Send a copy of
the Treasurer page to your district treasurer.
- Address registration questions to Sheri
Ketaneh:

- Changes (rooming, contests, participants,
etc.) are permitted until February 25.
- Do not submit a second registration. All
corrections—additions or deletions—must be sent to Sheri. She will
confirm that the correction has been made.
- For each registered student, the advisor must
have an Attendance and Emergency/Medical Release form signed
by a parent or guardian and a school administrator. The form is
available on the website. The advisor will give the signed forms to
registration personnel.
